GENERAL AGED CARE SERVICES
Twilight Aged Care is constantly evolving to ensure we’re at the forefront of responsive ageing-in-place care services. This includes specific palliative, dementia, and respite care and the very best of residential comfort with a range of room and apartment options.
We are committed to developing personal care programs for all residents , partnering and consulting with you to deliver a person-centered care plan, focusing on quality of life, choice, and reablement.
Some of our basic care services include:
- Maintaining continence or managing incontinence
- Assistance with mobility and maintaining independence
- Medication management
- Assistance with access to medical and allied health professionals
- Rehabilitation support, falls and mobility assessments
- Exercise therapy
- Registered Nurses onsite 24 hours per day, 7 days a week.
Residents classified by the Australian National Aged Care Classification (AN-ACC) as High Care will be provided with the following at no additional cost:
- Toiletries
- Special bedding requirements (i.e. air mattress, mechanical bed, etc.)
- Continence pads or other aids
- Podiatry
- Physiotherapy
- Exercise therapy
- Oxygen therapy
PALLIATIVE /END OF LIFE CARE
Our palliative care service offers spiritual, emotional, and physical comfort, along with good communication with and support for the resident’s family/guardian during the final stages of illness and in the bereavement period. We are dedicated to working as a team with our residents, their families, and medical providers to deliver dignified care, optimum pain management, and comfort during end-of-life. This is in line with Twilight’s charter of compassion and concern for residents’ welfare.
Care is provided by a multi-disciplinary team of health professionals who are competent in assessing the needs of each patient and work in alignment with the choices of residents and families to fulfill their wishes.
DEMENTIA CARE
We are dedicated to utilising the latest research, education and practical experience to provide optimum dementia care for residents. We do this in a variety of ways, including keeping families/guardians and care staff updated on the latest study findings and recommendations for managing and living with dementia. We also have a full-time Dementia Specialist on staff to support families and residents, as well as to provide ongoing training to our teams. It is our mission to constantly educate ourselves and the community to ensure we can improve the quality of life for residents living with dementia.
RESPITE CARE
Each of our homes provide Respite Care. For planned respite, please, contact your chosen home. Our Facility Managers or Administration Officers will be happy to discuss respite with you at any time.
REGISTERED NURSE CARE
Twilight Aged Care provides personalised care with a Registered Nurse rostered on staff at each of our homes 24/7. Registered Nurses undertake medication management and complex healthcare in partnership and consultation with residents and their families.
